Housing Navigator

The Housing Navigator is responsible for assisting low‐income individuals/families and who are homeless, formerly homeless, or at-risk of becoming homeless. The Housing Navigator will work closely with community agencies, governmental and otherwise, to help identify and secure housing options to ensure that families gain and maintain permanent housing. They will provide culturally inclusive, trauma-informed programming and services and will incorporate best practice techniques into the program.

Case Manager

A Case Manager plays a critical role in providing comprehensive support and advocacy to individuals and families facing various challenges, such as medical, social, or behavioral issues. They collaborate with clients, healthcare professionals, social services agencies, and other stakeholders to develop and implement personalized care plans, ensuring that clients receive the necessary resources and support to improve their well-being and achieve their goals. This Case Manager position will require some traveling between our Fort Pierce and Vero Beach non-congregate shelters to work hands-on with clients. A reliable vehicles is required.

The Healthcare Navigator at Treasure Coast Homeless Services Council (the Council) plays a crucial role in assisting homeless individuals and families in accessing essential healthcare services. They serve as a guide and advocate, helping clients navigate the complex healthcare system, access medical resources, and improve their overall well-being. This role may require some travel as you assist clients across the served tri-county region of Indian River, St. Lucie, and Martin. Please be aware this is not a job considered in the medical field, and nursing degrees are not necesary. Experience in Case Management, especially medical case management are preferred.
